Directions for Demize Tryout On-Site Registration 

PLEASE READ THESE DIRECTIONS CAREFULLY 

• The following items must be brought with you in order to register on site on March 9 

or March 16: 

1. Completed Tryout Registration Form 

2. Waiver & Release Form  

3. Copy of your insurance card OR signed insurance waiver & release (bottom 

portion of waiver & release form) 

4. $40.00 registration fee  

• If any form/item from the list above is missing or filled out incorrectly, your 

information will be discarded and you will not be registered. 

• Checks made payable to: SPRINGFIELD DEMIZE 

• Extra forms will be available on site in the event that you forget one of the items above.
